Requests for information concerning programs within the department should be addressed to the specific division of the department. General requests for information may be made to: Public Information Officer, Iowa Department of Public Health, Lucas State Office Building, Des Moines, Iowa 50319-0075. The department’s home page on the Internet, www.idph.state.ia.us, also features a “contact us” option.
