Section 441.200.15. Requests for information for purposes other than research or treatment.

Requests for information from department adoption records for purposes other than research or treatment shall be made to the Department of Human Services, Division of Adult, Children and Family Services, Adoption Program, Hoover State Office Building, 1305 East Walnut Street, Des Moines, Iowa 50319-0114.

         200.15(1) The department shall not release identifying information from sealed adoption records. Adult adoptees, adoptive parents, birth parents, siblings or descendants of an adopted person, or legal representatives of any of the above shall be provided:

            a.               An adoption packet containing a sample affidavit for filing with the court,

            b.               Directions for filing the affidavit,

            c.               A list of county clerks of court,

            d.               The address of the bureau of vital statistics, and

            e.               Instructions on how to obtain the name of the Iowa county where the adoption was finalized, if necessary.

         200.15(2) An adopted person who was a resident of the Annie Wittenmeyer Home (Iowa Soldier’s and Sailor’s Home) may receive nonidentifying information from Annie Wittenmeyer records if the information is available.
